A Troubling Reality: Patient Safety and Cultural Barriers in Canberra's Healthcare System

In Canberra, a concerning situation has emerged where healthcare staff fear repercussions for raising patient safety concerns. The report, compiled by Chair Michael Walsh and his team, is based on consultations with over 170 stakeholders, including staff, representative groups, and patients. It also considers public data and numerous inquiries, reviews, and audits conducted over the past 15 years.

While acknowledging past initiatives to improve workforce culture, the report identifies ongoing cultural issues, particularly racism, as a significant challenge. Michael Walsh, leading the inquiry into the ACT health system, emphasizes the need for trust and transparency, stating that staff should feel confident about raising concerns without fear of consequences.

"Some staff have expressed concerns about cultural safety and racism within the health system," the report reveals. It further highlights opportunities to enhance the governance of quality and safety, with a focus on improving communication for patients on waiting lists.

The report also addresses the issue of lengthy wait times for surgeries and procedures, noting that better communication is needed to manage patient expectations. Additionally, it suggests that attempts to reduce wait times in surgery and emergency departments, while welcomed, could have benefited from more collaborative implementation.

An independent review of the ACT public health system's workplace culture has exposed high levels of bullying and harassment, along with poor complaint handling systems. Despite efforts by successive governments to address these issues, the report emphasizes that workplace culture remains a critical factor affecting staff well-being and their ability to provide safe and effective care.

"Stakeholders have raised the need to improve clinicians' confidence in escalating safety concerns," the report states. It goes on to say that some clinicians have left the ACT health system due to their belief in the need for improved clinical quality and safety arrangements, including better escalation processes.

Other concerns raised during consultations include the burden of administrative duties on clinicians and a lack of clarity around roles and responsibilities. There are opportunities to strengthen referral pathways and make better use of outpatient clinics and walk-in centers to ease the burden on hospitals.

The report also highlights issues with data availability and reliability. While the Digital Health Record has advanced healthcare delivery and improved quality and safety, concerns remain about how data is collected, stored, and used. The lack of standardized measures across states and territories makes it challenging to compare performance and outcomes of care.
